What Are Exosomes?
Exosomes are nanoscale extracellular vesicles — tiny membrane-enclosed particles between 40 and 160 nanometres in diameter — produced naturally by cells throughout the body. What Does IV Exosome Therapy Do?
Delivered intravenously, exosomes enter systemic circulation and can act on multiple organ systems simultaneously. The proposed mechanisms of action include:
- Immune modulation: exosomes have demonstrated strong immunoregulatory properties, helping to reduce chronic low-grade inflammation — one of the central drivers of biological ageing
- Cellular repair signalling: by delivering growth factors and regulatory RNAs to target tissues, exosomes can support regenerative processes at a cellular level
- Skin and tissue rejuvenation: studies in aesthetic medicine have documented improvements in skin quality, elasticity, and tone following exosome-based treatments

What Does the Research Show?
The evidence base for exosome therapy is still maturing, but it is meaningful. A 2024 systematic review and meta-analysis published in the Journal of Extracellular Vesicles (Van Delen et al., 2024) analysed 21 clinical trials assessing the safety and efficacy of human extracellular vesicle-based therapy. The review found that EVs demonstrate low immunogenicity and an ability to cross biological barriers — properties that make them particularly suited to systemic delivery. Importantly, the safety profile across included studies was favourable.
In aesthetic medicine specifically, a systematic review and meta-analysis published in Aesthetic Surgery Journal (2024) evaluated exosome-based therapies across human clinical trials. The review documented evidence supporting exosome efficacy for facial rejuvenation, skin quality improvement, and tissue repair — with an emerging body of data on the anti-ageing mechanisms at play.
A further review in the same journal noted that GMP-compliant exosome preparations are increasingly positioned not as supplementary agents but as primary therapeutic tools in next-generation regenerative protocols.
As with any emerging therapy, I want to be clear with my patients: the evidence is promising and growing, but this is not a field where we have decades of long-term data. I offer IV exosome therapy as part of a scientifically grounded, individually tailored longevity programme — not as a standalone cure-all.
